PC GAPS Case Manager/MNsure Navigator

About This Position

This position supports the Preferred Communities (PC) GAPS and MNsure programs at IIMN.  The PC GAPS aspect of this position works with ORR-eligible populations and the Refugee Services team to provide clients with case management services.  This includes creating service plans to address clients’  needs; submitting benefits applications; providing transportation; and sending referrals for community resources.  The goal of the PC GAPS program is for clients to be on the path to self-sufficiency within 90 days of enrollment in the program.  This role will also support IIMN’s MNsure team by helping New Americans assess eligibility and apply for a health insurance plan.  This entails becoming a certified MNsure Navigator through the State of Minnesota’s training program, department having one-on-one appointments with community members, and engaging in community outreach.

$50,000 – $52,000, depending on experience

Primary Responsibilities:

PC GAPS Duties and Responsibilities:

  • Meet clients and assess their needs and short-term goals.
  • Referrals to resources and service providers such as IIMN’s housing team, English classes, and employment services.
  • Assist with applications for public benefits such as SNAP, cash assistance, childcare, and healthcare.
  • Arrange appointments, transportation, and interpreters for public assistance, medical visits, social security office, school enrollment, and other necessary appointments.
  • Document tasks in case notes in an organized and timely manner and complete necessary and required paperwork for each case.
  • Work with Program Manager to develop, implement and analyze client feedback in accordance with Equity Plan and use data collected from clients to improve program implementation.
  • Gather data and provide information to Program Manager for reports to USCRI.
  • Perform outreach and connect with community organizations to build a network of resources to address client barriers.

MNsure Navigator Duties and Responsibilities:

  • Complete and submit MNsure applications for eligible applicants, including IIMN students and clients.
  • Attend initial and annual MNsure Navigator training.
  • Design and update descriptive program material.
  • Distribute and present program materials to IIMN’s students in class or at community outreach events.
  • Work with team to screen all IIMN students for health insurance eligibility.
  • Educate MNsure clients on the proper use of their health insurance.
  • Maintain MNsure Outreach client database.
  • Track client enrollment details to ensure accurate state reimbursement payments.
  • Write and submit monthly reports for MNsure Grant and assist grant writer with new grant applications.
  • Meet regularly with MNsure Outreach Grant supervisor.

General Duties and Responsibilities:

  • Attend required IIMN staff meetings.
  • Other duties as assigned.

Experience and Qualifications

  • Must be capable of working in a fast-paced, multicultural office environment, take initiative, and be able to handle and prioritize multiple assignments at once.
  • Bachelor’s degree or relevant work experience in a related field preferred
  • Commitment to the use of strengths-based client approaches, and to supporting families in their journeys to becoming confident, independent, and self-sufficient
  • Bilingual in English and Spanish highly preferred; ability to speak other common client languages including Arabic, Somali, Burmese, Karen, Oromo, Swahili, or French desired
  • Commitment to the human rights and general welfare of asylum seekers, refugees, and immigrants
  • Valid driver’s license and access to own transportation required. Must be willing to transport clients in personal vehicle.
  • Ability to work some late nights and weekends, as needed
  • Strong organizational skills and attention to detail including comprehensive case noting
  • Skilled in problem solving and able to independently address issues as they arise.
  • Experience with Microsoft Office applications.
